It's not explained at all. Whoever rebuilt him gave him new clothes.

Not only that but it looks like they tried to fix most of his head and he had no damage to his one hand.

My guess is that he got an entirely new Good Guy body and his head was somewhat refurbished. Or he is a new Good Guy altogether and Tiffany made his face stitched on purpose.

This would make no sense to do but I can see her doing it for 2 reasons.

1. She recognizes and identifies with Chucky looking like this.

2. To annoy him and make his job a little harder. Kinda like her way of saying "Thanks asshole" for the times he's killed her.
